ELIGIBILITY: The SparkPeople Name the Cookbook Contest for individuals (the “Contest”) is open only to permanent legal U.S. residents physically residing in the fifty (50) United States of America and the District of Columbia. This contest is subject to all applicable federal, state, local laws and regulations. Void where prohibited or restricted by law. HOW TO ENTER: Post your title ideas in the comments section of the dailySpark blog post about the contest. NOTE: Only titles selected by SparkPeople to be in the top 10 at the end of the Contest Period will be considered for the prizes. JUDGING: Each eligible entry in the Contest will be judged by Sponsor and the final title chosen by the Sponsor's Publisher, whose decisions are final and binding on matters relating to this Contest. Prize winners of the Contest will be selected based on a combination of factors. One winner will be selected from 10 semifinalists. DURATION: The contest will run from now until 2 p.m. EST on Wednesday, Jan. 12. PRIZES: All 10 semi-finalists will win a personally autographed copy of the cookbook after its publication in fall 2011. The winner will be included in the cookbook's acknowledgments and will receive $100 to spend in the SparkPeople Store. All taxes are the responsibility of the winner. Winner will be required to provide his/her social security number. Winner is advised to consult with his/her accountant, lawyer or other financial adviser regarding specific tax implications. No substitution or transfer of prize by a winner is permitted. Limit one prize per person/household/email address. NOTIFICATION: Winners will be notified by e-mail. Winner will be required to sign and return an Affidavit of Eligibility, Liability Release and where legally permissible, a Publicity Release within 14 days following the date of first attempted notification. Failure to return the documents within 14 days of receipt will result in forfeiture of the prize and Sponsor will award it to the next ranking entrant. GENERAL CONDITIONS: By participating in this Contest and submitting an entry, entrants agree to be bound by these Official Rules and the decision of the Sponsor. By entering the Contest, winner agrees to waive any and all claims against the Sponsor, its affiliates, employees, agents, and representatives for personal injury or loss that may occur from the participation in this contest or from the receipt or use of any prize, and grant to Sponsor the right to publicize such winner's name and/or likeness without additional compensation or consideration unless prohibited by law. Sponsor, its affiliates, employees, agents and representatives, or any telephone network or internet service providers are not responsible for incorrect, inaccurate transcription or loss of entry information for any reason whatsoever. If, in the Sponsor's exclusive judgment, this promotion becomes compromised in any way, Sponsor reserves the right to withdraw the promotion offer, cancel any method of entry, to void any entries submitted fraudulently or select winners from all eligible entries received prior to act that compromised the promotion.
